for Enrolment Form. To save time you can print this out and
fill it out before you come to the office.
First, have you got Acrobat Reader?

You'll need a copy of Adobe's Acrobat Reader
which is available as a FREE download from Adobe. You can
get your copy of this now by clicking and downloading from
this link. Then install it. It will take just a couple of
minutes and you'll be able to use if for many other .PDF files
you find on the web.

Enrolment Procedures
- Parents should enrol their children up to a year before
their fifth birthday. Parents need to come to the school
office and fill out an enrolment form. They should bring:
- bring their child's birth certificate (if born
in NZ), or,
- passport showing his/her residence permit (if
born outside NZ)
- immunisation certificate for all children born
after January 1995 need to have an which is in the back
of your child's Plunket book (filled in and signed by your
doctor)
- Proof of address if you are living in our school
zone e.g. a copy of a current electricity or telephone account.
After the enrolment has been processed by the Principal a
letter of acceptance is sent to you.
Three pre entry sessions take place three weeks before your
child starts school on a Wednesday from 11.15 to 12.30. The
second and third visits your child will be able to bring his/her
lunch and play at lunchtime. A letter is sent to parents about
a month beforehand explaining pre entry.
Medicine Consent Form
If you child requires medication to be
given during school hours, a Medicine Consent Form must be
filled out and given to the school along with the medication.

under NAG 5 Health & Safety, if you require further information.
